Diabetes Prevention Program
You can reduce your diabetes risk and gain tools for healthy living and lasting weight loss with this free* program
Prevention: The strongest weapon against diabetes
If you have pre-diabetes, you’re at a higher risk for diabetes unless you take action now. But maybe you’re not sure what steps to take next. Studies show that those with pre-diabetes can delay, and often PREVENT, the development of type 2 diabetes. It’s all about making simple changes in your life. That often includes only moderate activity and a small amount of weight loss. The Diabetes Prevention and Control Alliance is working with the Y to help you make it happen.
Brought to you by the Y, Diabetes Prevention Program is designed to help those with pre-diabetes avoid type 2 diabetes. In this 16-week group-based program - at one hour a week - you can:
• Learn how to lose weight and keep it off.
• Explore fun, easy ways to change your life.
• Get the motivation you need in a small group setting, with personal attention from a trained coach.
Studies show that as little as a 5 to 7% loss in body weight can reduce the chances of those with pre-diabetes from getting diabetes by 58% when using this program! Little changes truly do add up. This is the support to get you there.

If you do not have health insurance through Pima County, the Diabetes Prevention Program may be available to you for a small fee or through other coverage.
Program Goals include:
• Reduce body weight by 7%
• Increase physical activity to 150+ minutes per week
Monthly support meetings are offered after you have completed the initial 16 core sessions. |
Are you eligible to participate?
To qualify for the YMCA Diabetes Prevention Program, you must be overweight (BMI >25)* and at high risk for developing type 2 diabetes or have been diagnosed with prediabetes.
Are you at risk for developing diabetes? You may be at risk if two or more of the following are true:
• You are 45 years of age or older
• You are fairly inactive; You are NOT physically active more than two times a week
• Your blood pressure is 140/90 or higher, or you've been told you have high blood pressure
• You have been told that your cholesterol levels are not normal
• You have or had a parent or sibling with diabetes
• You have had gestational diabetes (during pregnancy), or gave birth to a baby weighing more than 9 pounds
If you have been diagnosed with pre-diabetes, you may be eligible for this program if one or more of the following is true:
• Your fasting glucose is 100-125 mg/dl
• Your 2-hour (75 gm glucola) Plasma Glucose is 140-199 mg/dl
• Your random/casual blood glucose is 140-199 mg/dl
• Your A1c is 5.7-6.4%

This program is not offered by Employee Wellness, but through UnitedHealthcare and the Diabetes Prevention and Control Alliance. You must be insured under one of Pima County's UnitedHealthcare plans to participate at no cost. (If you plan to drop coverage before the end of the program, your program fee will not be covered.)


Diabetes Control Program
You can save money, reduce your risk of complications and gain important knowledge with this free* program
This valuable, free* program includes:
An initial one-hour appointment including:
• Biometric Labwork including A1c and cholesterol
• Basic ADA physical examination
• Medication review
• Important health information
Quarterly follow-up appointments including labwork (or more often if recommended) for up to one year.
All appointments are conducted in a private setting with a specially trained pharmacist who will communicate and share lab results with your doctor.

A more convenient way to control diabetes
Managing diabetes is important, but it’s not always easy. There’s plenty to do, like make changes to your diet. Exercise regularly. Coordinate medicines. Monitor blood sugar. It can be pretty overwhelming. But it doesn’t have to be such a challenge — it can be more convenient.
That’s why the Diabetes Prevention and Control Alliance (DPCA) is making it easier to manage your diabetes, particularly your medications. The Diabetes Control Program (DCP) was made just for you. The DPCA has brought together a group of highly qualified pharmacists who are specially trained to help you manage your diabetes.
Your pharmacist can:
• Help you understand your treatment.
• Show you the best way to take your medicines. And make
you aware of side effects and interactions.
• Offer ongoing education and give healthy lifestyle tips.
• Help coordinate routine care with your doctor.
• Help you monitor weight, blood pressure and lab tests.
• Provide review for early signs of diabetes complications.
Working with your pharmacist can be a convenient way to manage your diabetes and related medicines. It’s as easy as stopping at your local drug store.

Currently, the Diabetes Control Program in Tucson is available at Albertsons Pharmacies at many of their Tucson locations. Additional pharmacies in Tucson will soon be participating. If you have diabetes, contact the Diabetes Prevention and Control Alliance at 1-800-650-2885 to sign-up.
How can this program save you money?**
• Personalized appointments like this could cost you hundreds of dollars outside of this program
• Important A1c and cholesterol tests are covered at no additional out-of-pocket cost (quarterly when actively participating in the program)
• When your diabetes is well-managed, you may save on medication and testing supply costs as well as the high cost of treating complications
• If you consider time as money, you will save time by having your labwork done at the pharmacy rather than making a separate trip to a laboratory; and your pharmacist will communicate results with your doctor's office

Gift Card Reward*
for completing the Diabetes Prevention or Control Program
Employees may earn a $60 Target gift card when enrolling in one of the programs and completing the gift card reward program requirements which include:
Prevention: Attend all 16 sessions and achieve a 5% or greater weight loss during the program.
Weight loss must be achieved either by the end of the 16-week program, or within 3 months of the end of the 16-week program while participating in monthly follow-up sessions. Make-up sessions may be counted when missing a regularly scheduled class.
Control: Attend 4 sessions and lower your A1c at least 1% during the program; or maintain an A1c level of under 7% during the program.
Attendance at only 2 to 3 sessions is acceptable if the 3rd and 4th are not recommended by the program’s pharmacist.
To earn your gift card:
• Enroll in one of the programs
• Register with Employee Wellness for the gift card reward: Registration Form
• Complete the program requirements listed above
• Submit your completion certificate, signed by the YMCA facilitator or pharmacist, to receive your gift card (Certificate provided by Employee Wellness)
*Gift cards limited to the first 250 employees to register and complete the program
