If you enroll in the Choice Fund/HSA, in addition to an Insurance Enrollment Form, you will also need to complete an HSA application and HSA Payroll Deduction Authorization form.

In addition to paying the full premium cost for all levels of coverage in this plan, Pima County will contribute funds each pay period to your HSA as follows:
