Life InsuranceGroup #s (Term 33359 / AD&D 33360) Administered through: Basic Life Insurance Coverage. Pima County provides $10,000 of Basic Term Life Insurance and $10,000 of Basic Term Accidental Death and Dismemberment (AD&D) Insurance for all insurance eligible employees at no cost to the employee. Supplemental Life Insurance Coverage is available and fully payable by eligible employees for as much as four times the employee's salary, but not to exceed $500,000. An additional AD&D policy may be elected for eligible employees and families. Dependent Life Insurance Coverage for the eligible employee's spouse and eligible children may also be elected and is fully payable by the employee. If additional life insurance coverage is not elected during the first 31 days of employment, you will be required to complete an Evidence of Insurability (EOI) form (medical history statement) for any insurance coverage increase requested during open enrollment. Premiums are based on your age and wage.
