Domestic Partner

If you wish to insure a domestic partner, you must complete a Domestic Partner Affidavit each Fiscal Year.

Domestic Partner Affidavit (for insurance related purposes, as well as use of sick leave and humanitarian leave)

Domestic Partner Affidavit (for non-insurance related purposes, only the use of sick leave and humanitarian leave)

Keep in mind, a Domestic Partner, unless a qualified tax dependent, is not an eligible dependent in a Flexible Spending Account or Health Savings Account. As such, if you participate in either account, you will need to manage your contributions and distributions accordingly.
